Standard Operasional Prosedur (SOP) Permohonan Surat Kegiatan Akademik dan Kemahasiswaan

Dalam rangka tertib administrasi serta percepatan pelayanan kegiatan kemahasiswaan di lingkungan FMIPA Universitas Negeri Semarang, maka perlu  kami sampaikan  Standar   Operasional  Prosedur  (SOP) Layanan Permohonan Surat Kegiatan Akademik dan Kemahasiswaan yang dilayani di Administrasi Akademik dan Kemahasiswaan FMIPA UNNES sebagai berikut:

  6. Surat Izin Pelaksanaan FMIPA Prigel/Magang Mandiri
  7. Surat Tugas/ Surat Izin Mengikuti Kegiatan
  8. Surat Izin Observasi Mata Kuliah
  9. Surat Pengantar KKL
  10. Surat Keterangan Lulus (SKL)
  11. Surat Pengantar Kehilangan KTM
  12. Legalisasi ijazah dan transkrip

  1. Prosedur Layanan
  1. Permohonan surat menyurat yang  dapat  dilayani  oleh  Fakultas hanya :

  • Dosen/ Tendik FMIPA
  • Mahasiswa FMIPA (D3/S1/S2/S3)
  • Alumni FMIPA

  1. Alur  Permohonan Surat

  1. Dosen

  1. Mengajukan surat permohonan kepada Dekan FMIPA dan disampaikan melalui Bagian Administrasi Umum, SDM dan Kerjasama FMIPA UNNES atau dapat melalui link berikut:  s.id/PermohonanSuratTugasDosenTendik
  2. Melengkapi berkas pendukung yang dibutuhkan
  3. Melakukan pengecekan secara berkala atas permohonan yang telah diajukan melalui email atau telegram (untuk permohonan Surat Tugas/Surat Izin Kegiatan)
  4. Apabila sampai dengan 3 (tiga) hari kerja sejak tanggal pengajuan masih belum mendapatkan balasan surat, dapat melakukan konfirmasi langsung di Administrasi Akademik dan Kemahasiswaan FMIPA (Gedung D12 Lantai 1)

CATATAN:

  1. Standard penyelesaian adalah selama 3 (tiga) hari kerja sejak surat terdisposisi/ tersampaikan di Bagian Administrasi Akademik dan Kemahasiswaan
  2. Pengambilan surat yang sudah jadi bisa dilakukan langsung di Bagian Administrasi Akademik dan Kemahasiswaan (Gedung D12 Lantai 1) atau akan dikirimkan melalui email/no HP yang tercatat pada surat permohonan
  1. Mahasiswa

  1. Mengisi permohonan sesuai template surat yang dibutuhkan melalui MYUNNES-STUDENTS untuk surat-surat berikut:

  1. Layanan Surat Izin Penelitian.
  2. Layanan Surat Aktif Kuliah          
  3. Layanan Surat Observasi.
  4. Layanan Permohonan Validator Ahli
  5. Layanan Surat Keterangan Lulus
  6. Layanan Surat Kehilangan KTM
  7. Layanan Surat Rekomendasi Beasiswa
  8. Layanan Surat Observasi Mata Kuliah
  9. Layanan Surat Izin  Pelaksanaan PRIGEL/ Magang Mandiri
  10. Untuk Layanan Cetak Riwayat Studi Mahasiswa, silahkan mengajukan permohonan ke Administrasi Akademik dan Kemahasiswaan melalui https://s.id/RiwayatNilaiAkademik
  1. Melengkapi berkas pendukung yang dibutuhkan
  2. Melakukan pengecekan secara berkala atas permohonan yang telah diajukan baik melalui aplikasi MYUNNES-STUDENTS
  3. Apabila sampai dengan 3 (tiga) hari kerja sejak tanggal pengajuan masih belum mendapatkan balasan surat, dapat melakukan konfirmasi langsung di Administrasi Akademik dan Kemahasiswaan FMIPA (Gedung D12 Lantai 1)

Nb. Apabila terjadi kendala dalam aplikasi, maka Mahasiswa dapat mengajukan surat permohonan langsung kepada Dekan dan disampaikan melalui Bagian Administrasi Umum, SDM dan Kerjasama untuk dapat didisposisi. Bagian Akademik dan Kemahasiswaan akan memproses permohonan dalam 3 (tiga) hari kerja terhitung sejak surat terdisposisi/ tersampaikan di Bagian Akademik dan Kemahasiswaan

  1. Alumni FMIPA

  • Layanan Pembuatan Surat Keterangan Alumni.

Syarat dan Ketentuan Pengajuan:

  1. mengajukan surat secara tertulis kepada Dekan FMIPA UNNES
  2. melampirkan fotocopy ijazah atau transkrip
  • Layanan Legalisasi Ijazah dan Transkrip secara Offline.

Syarat dan Ketentuan Pengajuan:

  1. menyerahkan fotocopy ijazah/transkrip yang akan di legalisasi(maksimal masing-masing 5 lembar)
  2. menunjukkan ijazah/transkrip asli
