MEKANISME PENGAJUAN PENGUNDURAN DIRI SEBAGAI MAHASISWA FAKULTAS MATEMATIKA DAN ILMU PENGETAHUAN ALAM UNIVERSITAS NEGERI SEMARANG

Dalam rangka tertib administrasi serta percepatan pelayanan akademik dan kemahasiswaan di lingkungan FMIPA Universitas Negeri Semarang, maka perlu kami sampaikan Mekanisme Pengajuan Pengunduran Diri Sebagai Mahasiswa FMIPA UNNES yang dilayani di Administrasi Akademik dan Kemahasiswaan FMIPA UNNES sebagai berikut:

A. Persyaratan Mahasiswa Yang Mengajukan Pengunduran Diri Sebagai Mahasiswa

  1. Masih berstatus aktif sebagai mahasiswa/ tidak berstatus DO (Drop Out).
  2. Sudah menyelesaikan semua tanggungan terkait keuangan, peminjaman buku di perpustakaan maupun peminjaman peralatan di laboratorium.

B. Alur Permohonan Surat

Mahasiswa

  1. Mengisi surat permohonan pengunduran diri dan surat pernyataan sesuai contoh surat yang telah disediakan melalui link berikut: https://s.id/SuratPengunduranDiriMahasiswaFMIPA
  2. Meminta persetujuan koordinator prodi dan orang tua/wali dari  mahasiswa yang bersangkutan.
  3. Melengkapi berkas pendukung yang dibutuhkan.
  4. Mengajukan surat permohonan dan surat pernyataan beserta lampiran berkas pendukung ke Bagian Administrasi Akademik dan Kemahasiswaan FMIPA untuk dapat diproses lebih lanjut.
  5. Surat permohonan kepada Wakil Rektor Bidang Akademik dan Kemahasiswaan akan dikirimkan oleh Bagian Administrasi Akademik dan Kemahasiswaan dalam waktu maksimal 3 (tiga) hari kerja sejak surat didisposisi di Bagian Administrasi Akademik dan Kemahasiswaan FMIPA. Apabila sampai dengan 3 (tiga) hari kerja sejak tanggal pengajuan masih belum mendapatkan tembusan surat yang ditujukan kepada Wakil Rektor Bidang Akademik dan Kemahasiswaan, mahasiswa dapat melakukan konfirmasi langsung di Bagian Administrasi Akademik dan Kemahasiswaan FMIPA (Gedung D12 Lantai 1 FMIPA UNNES).
  6. Mahasiswa bisa melakukan pengecekan secara berkala pada link berikut: https://siradi2.unnes.ac.id/ melalui menu LACAK SURAT untuk dapat melihat proses suratnya dengan menggunakan Nomor Surat yang tercantum dalam surat permohonan kepada Wakil Rektor Bidang Akademik dan Kemahasiswaan. Apabila posisi surat sudah sampai di Seksi Layanan Akademik dan Kemahasiswaan DAKK, maka mahasiswa dapat melakukan konfirmasi ke Seksi Layanan Akademik dan Kemahasiswaan DAKK (Gedung Rektorat Lantai 1) untuk proses berikutnya.
