Berdasarkan Surat Kementerian Pendidikan Dasar dan Menengah Direktorat Jenderal Guru Tenaga Kependidikan, dan Kependidikan Guru Nomor 0406/B2/GT.00.08/2025 tentang Pelaksanaan Pendidikan Profesi Guru (PPG) bagi Guru Tertentu (Dalam Jabatan) Tahap 1 Tahun 2025, maka kami informasikan kepada seluruh Calon Mahasiswa PPG Guru Tertentu (Dalam Jabatan) Tahap 1 Tahun 2025 Universitas Negeri Semarang bahwa kegiatan lapor diri di LPTK sesuai edaran dari GTK Kemdikdasmen akan dilaksanakan mulai tanggal 22 Mei 2025 sampai tanggal 1 Juni 2025 secara online di tempat masing-masing. Silahkan untuk masing-masing peserta melengkapi terlebih dahulu berkas sesuai yang disyaratkan (lengkap). File/berkas persyaratan yang wajib disiapkan untuk kemudian diunggah ke dalam sistem datapokok (link sistem datapokok akan kami sampaikan melalui grup telegram (mohon menunggu perkembangan informasi di grup telegram).

silakan sambil menunggu perkembangan informasi di grup telegram, Bapak/Ibu dapat melengkapi berkas yang wajib disiapkan untuk lapor diri.

Berkas persyaratan Lapor Diri:

  • Scan Asli Pakta Integritas yang ditandatangani peserta di atas materai Rp. 10.000. Silakan lembar Pakta Integritas dapat mengunduh melalui sistem SIMPKB saat Bapak/Ibu melakukan konfirmasi kesediaan. Tanggal Pakta Integritas disesuaikan dengan tanggal saat Bapak/Ibu melakukan Lapor Diri. Tipe file PDF (maksimal 1 MB) dengan format penulisan File: PaktaIntegritas_Nama Lengkap_Bidang Studi PPG. Contoh penulisan: Pakta Integritas_Joko Sriyanto_PGSD.
  • Scan Asli Biodata Mahasiswa sesuai format PDDIKTI. Tipe file PDF (maksimal 1 MB) dengan format penulisan File: Biodata_Nama Lengkap_Bidang Studi PPG. Contoh penulisan: Biodata_Joko Sriyanto_PGSD.
  • Scan Asli Surat Pernyataan Izin Pimpinan/Kepala Sekolah (Format dapat diunduh di SIMPKB). Tipe file PDF (maksimal 1 MB) dengan format penulisan File: IzinPimpinan_Nama Lengkap_Bidang Studi PPG. Contoh penulisan: IzinPimpinan_Joko Sriyanto_PGSD.
  • Scan Asli ijazah S1/ DIV atau yang sudah dilegalisir oleh universitas yang menerbitkan ijazah. Tipe File PDF (maksimal 1 MB) dengan format penulisan File: Ijazah_Nama Lengkap_Bidang Studi PPG. contoh penulisan: Ijazah_Joko Sriyanto_PGSD.
  • Scan Asli Transkrip Nilai yang sudah dilegalisir oleh universitas yang menerbitkan transkrip nilai. Tipe File PDF (maksimal 1 MB) dengan format penulisan File: Transkrip_Nama Lengkap_Bidang Studi PPG. contoh penulisan: Transkrip_Joko Sriyanto_PGSD.
  • Scan Asli Kartu Identitas KTP. Tipe File PDF (maksimal 1 MB) dengan format penulisan File: KTP_Nama Lengkap_Bidang Studi PPG. contoh penulusan: KTP_Joko Sriyanto_PGSD.
  • Soft File Pas Foto Berwarna Background Warna Merah/ Latar Belakang Warna Merah berukuran 3 x 4 (tipe JPG Maksimal 1 MB) dengan format penulisan File: Foto_Nama Lengkap_Bidang Studi PPG. contoh penulisan: Foto_Joko Sriyanto_PGSD.

Ketentuan Foto:

  • Foto berwarna dengan latar belakang merah.
  • Bagi mahasiswa pria memakai jas hitam, hem putih dan dasi hitam (bukan dasi kupu-kupu), hem putih, tidak memakai kacamata, tidak memakai kopyah/peci/topi
  • Bagi mahasiswa wanita memakai jas hitam, hem/blus putih, tidak memakai kacamata, kerudung hitam polos bagi yang berkerudung (berdasi bukan dasi kupu-kupu)
  • Posisi badan menghadap depan, posisi seimbang kiri-kanan, bagian atas kepala tidak berada tepat di bawah batas foto (ada sisa), dan proporsional untuk ukuran pasfoto
  • Format foto JPG max 100kb
  • Mengingat ini adalah untuk dokumen formal dan untuk serdik mohon membuatnya di studio foto tidak disarankan menggunakan kamera handphone. Jangan melakukan repro dengan scanner atau mengambil ulang gambarnya (memfoto) dari pasfoto cetak.

Ketentuan tambahan

  • Scan Asli SKCK (minimal dikeluarkan oleh Polsek). Tipe File PDF (maksimal 1 MB) dengan format penulisan File: SKCK_Nama Lengkap_Bidang Studi PPG. contoh penulusan: SKCK_Joko Sriyanto_PGSD.
  • Scan Asli Surat Keterangan Sehat (dari Puskesmas/RSUD setempat). Tipe File PDF (maksimal 1 MB) dengan format penulisan File: Surat Sehat_Nama Lengkap_Bidang Studi PPG. contoh penulisan: Surat Sehat_Joko Sriyanto_PGSD.
  • Scan Asli Surat Bebas Napza (boleh dari BNN, Kepolisian, Puskesmas, maupun RSUD setempat). Tipe File PDF (maksimal 1 MB) dengan format penulisan File: BebasNapza_Nama Lengkap_Bidang Studi PPG. contoh penulisan: BebasNapza_Joko Sriyanto_PGSD.
  • Scan Asli NPWP (bagi yang memiliki). Jika tidak memiliki silahkan tulis di mocrosoft word tulis “Tidak Memiliki NPWP” kemudian disave dengan format PDF. Tipe File PDF (maksimal 1 MB) dengan format penulisan File: NPWP_Nama Lengkap_Bidang Studi PPG. contoh penulusan: NPWP_Joko Sriyanto_PGSD.
